- Your institution
- An indication if you will be attending: the EPUG-UKI meeting (Monday 19 October); the EPUG-UKI-SMUG Dayschool (Tuesday 20 October) or both events
- Any special dietary requirements
The cut-off date for registration is Monday 12 October 2009.
